It operates the only HIV Medical Home in the United States, as designated by the Centers for Medicare and Medicaid Services. This model of care integrates a range of health and social services, including medical, dental, mental health, and pharmacy services, along with case management and social support from a dedicated team of professionals. In addition to treatment, Vivent Health provides innovative prevention services aimed at helping at-risk individuals maintain their HIV-negative status. With 15 locations across Colorado, Missouri, Texas, and Wisconsin, Vivent Health serves tens of thousands of individuals at risk for HIV and supports over 8,000 people living with HIV each year.
